This last week at County has been a decent one actually.  It's been frustrating trying to get things done for my patients, and yet I feel I've been able to make a small difference to some of them.  As for medicine, two weeks ago, I would have said boy am I ever second guessing that choice.  Of one thing I am now convinced, I want to go to programs with closed ICU's.  The mixed picture just adds that extra element of stress that can really take away from learning.  Musing over my experience this month so far though, I actually now feel much more comfortable taking care of ICU pts.  For example, I've had a couple pts come in w/ DKA, and learned a lot from each.  I'm still pretty much off the low end of the scale in terms of Intensivist competency, but with experience comes learning.&lt;br /&gt;&lt;br /&gt;It's getting cool again, and I love it.  Can't wait to go to Salt Lake City and then Alberta at Xmas!  family, snow, skiing! It was a doozy!  I kept getting these dermatology questions that I had no clue on.  something like this...  34 year old female comes in complaining of rash for 3 days and yada yada...  whats the mechanism of the drug that is the best treatment for this condition?  WHAT??!!  I'm still back on what in the world is this..  Sadly I quickly discovered that was the one area I did NOT study very well.  Oh well, I had 3 q's on Cluster headache.  Cha ching!  I took those to the bank thank you very much...  Well I have another 8 hour whopper this coming Monday, but it shouldn't be too bad.  It will be nice to actually have some patient interaction again.  I've missed it. Thought about heading to the beach with the church, but traffic is catastrophic as everyone else in the inland empire has the same thought...  get to the beach!  I'm interested in seeing the ex-vice president's documentary.  I have been told it is rather interesting, I'll have to find it.  I floated the idea of heading up to Death Valley, out of sheer morbid curiousity.  How hot would that feel?!  And how about baking an egg on the car hood?  However, the idea was met with a mix of revulsion and sarcasm.  Maybe some other day...  So for now, the title of this blog certainly reveals my bias. Too little? Too much?  usually too little...  I've had to resort to a little diphenhydramine to help the sandman.  Perspective is everything.  I'm feeling nicely manipulated by the media tonight.  It's terrible what's happening in the middle east right now and I wanted to get a little TV news coverage.  BBC seemed to report exclusively from the Lebanese point of view, and having switched to ABC news, they reported from nothing but the Israeli perspective.  Sad from both angles..  I do think Israel has been excessive, but what else can you do from the air.  It's only going to get worse.
